The Attack on Carson Wolf was an ambush on Homeland Security agent Carson Wolf orchestrated by the Punisher in order to extract the information about David Lieberman.
Background[]
When William Rawlins discovered that Dinah Madani had acquired the footage of the execution of Ahmad Zubair by the Cerberus Squad, he ordered Carson Wolf to find the person who leaked the footage. Wolf found out that Madani's source was the mysterious hacker, nicknamed Micro, and the New York Bulletin was going to write an article about him. Wolf personally met with Mitchell Ellison and coerced him into not writing an article, noting that it could interfere with the current Homeland Security operation.
Rawlins and Wolf then discovered that Micro's real identity is an NSA analyst David Lieberman, so Rawlins ordered Wolf to eliminate him. Together with the group of Homeland Security agents, Wolf tracked Lieberman down and chased him across the town before shooting him in the chest, seemingly killing him. Wolf twisted the narrative, deeming Lieberman as a domestic terrorist and removed most of the details about him from a public access. He also pulled Madani out of Afghanistan who started her own investigation of Zubair's death after the footage she acquired was stolen from her.[2]
In reality, Micro survived the attack and went off the grid, planning to take down Rawlins and return to his family, so he tried to get in contact with Frank Castle. In order to find Micro and learn about his motives, Castle asked Karen Page for help and she discovered some bits of information about David Lieberman and Wolf's involvement. Page informed Castle about her findings and Castle, knowing about Wolf's involvement into the Operation Cerberus, decided to visit him.[1]

"And Lieberman, what about Lieberman? Who's he?"
"We thought you were the one that sent him the tape. Now close your eyes, Frank. I'm gonna do you a favor. You're gonna see your family real soon." - âCarson Wolf and Punisher[src]
Wolf went home and approached his fridge to get himself a snack, being immediately attacked by a masked intruder after closing it. Wolf defended himself against the intruder until he was knocked unconscious and tied to a chair.
The intruder then interrogated Wolf after he regained consciousness, telling the former that he'd been tortured in Guantanamo Bay and that nothing would break him. The intruder then shot Carson in the leg and stuck his finger in the bullet hole. Amidst interrogation, Carson broke free and got the upper hand on his interrogator, pointing his gun at him. Carson took off the intruder's mask to reveal it was Frank Castle. Carson shoots the gun and it clicks, with Castle replying that it's empty, with another battle ensues. Castle gained the upper hand, putting Carson into a headlock and snapping his neck, killing him.[1]

Carson Wolf's dead body was then discovered in his house and Dinah Madani and Sam Stein were called at the scene. The New York City Police Department and the FBI attempted to take over the investigation of Wolf's murder, however, the Homeland Security took the case in their own hands. Stein personally checked Wolf's bank records and discovered six illegal offshore accounts with $30,000,000 owned by Wolf.[2]
As Wolf's illegal activity was revealed, the Office of the Inspector General is taking over the investigation into Wolf's death and his corruption which negatively affected the whole agency. Due to Rafael Hernandez's recommendations, Homeland Security assigned Madani to replace Wolf as the SAC in New York. During the operation at the Red Hook Pier, where Madani encountered Frank Castle himself, he told Madani that he killed Wolf and confirmed his corruption.[3]
