In the base game of Fallout: New Vegas, implants are available to purchase from Doctor Usanagi, a doctor with the Followers of the Apocalypse who heads the New Vegas medical clinic located slightly east of the Crimson Caravan Company compound outside of Freeside during the unmarked quest Harder, Better, Faster, Stronger. Speaking with her about available implants, she will offer a nine-part series of physical implants that behave in the same manner as taking ranks of the Intense Training perk, permanently raising a single SPECIAL attribute by 1 level (much like the function of bobbleheads in Fallout 3). One cannot purchase a physical implant if the corresponding SPECIAL attribute is already at the maximum level of 10. Usanagi will also mention two special implants that can impart the effects of passive HP regeneration and a permanent increase to one's Damage Threshold.
Four special implants are available to purchase from the Sink Auto-Doc once its personality module is restored in the Old World Blues add-on.
Implant limit[]
The implant limit, i.e. the number of implants that can be installed by Dr. Usanagi, is equal to the player character's Endurance attribute level. Only the permanent base Endurance score is counted for the implant limit allowance; temporary boosts from consuming aid items and equipping apparel that increases Endurance are not counted. The permanent increase from purchasing the Endurance Implant does not increase the implant limit, whereas raising one's Endurance with ranks of Intense Training or with the 1-point bonus from whichever one of the four special perks gained at the end of the Lonesome Road add-on does allow for purchasing an additional implant. The four implants found in the Old World Blues add-on do not count against the implant limit.
Since the effects of the physical implants bought from Dr. Usanagi are counted as a permanent increase to their respective SPECIAL attribute, buying them to increase an attribute will affect the availability of perks that may require a certain attribute level.
Fallout: New Vegas[]
At the New Vegas medical clinic, Doctor Usanagi sells both physical implants to augment one's attributes, and two special implants to augment combat performance. Only one of each type can be installed. Once one pays the required caps amount to Usanagi, the screen will fade to black and a period of 3 in-game hours will elapse. Acquiring all 9 implants from Usanagi will require a minimum 9 Endurance attribute level and a total of 48,000 caps.

Old World Blues[]
In the Old World Blues add-on, four special implants can be purchased from the Auto-Doc in the Sink, after first acquiring and installing its personality module and also locating the implant holotapes around Big MT. Unlike the implants acquired from Usanagi, they can all be obtained regardless of the player character's Endurance level. Acquiring all 4 implants from the Auto-Doc will require a total of 50,000 caps.

- In normal mode, the PHOENIX Monocyte Breeder implant (regenerate 1 HP every 10 seconds) can effectively replace the need to sleep in order to regain HP. Waiting will restore 12 HP an hour, or 288 HP in 24 hours, meaning it will often only take a single day of waiting to restore health. This also works during fast travel.
- If the player character has an INT attribute level of 3 or below, a unique low-Intelligence option will be available with Usanagi, "You sell plants?" Choosing it will cause the doctor to offer the Intelligence implant for a reduced price out of pity, allowing one to buy that specific implant for 1,000 caps; the others will remain at the normal 4,000-caps price. Lowering the INT stat temporarily via chems or gear will unlock the dialogue option but won't generate the discount, even if one's INT is lowered before the first time speaking with Usanagi.
Behind the scenes[]
The quest name "Harder, Better, Faster, Stronger" is a reference to The Six Million Dollar Man, a late 1970s TV show about a former astronaut who suffers severe injuries and is augmented with bionic implants that give him superhuman abilities.
Bugs[]
If you are offered the low-Intelligence discount but do not purchase the Intelligence implant and then you leave the hospital, when you return and speak to Usanagi next time she will no longer offer this implant - discounted or otherwise.[verified]
