Skull Crusher can be found inside a chest at the Forge of Hilbongard in the Forgotten Vaults of Anudnabia, which is accessed through an entrance in the Omaren Ancestral Tomb (east of Sadrith Mora).

During this Imperial Cult quest, Lalatia Varian sends the Nerevarine to retrieve the Skull Crusher from Anudnabia.
The Museum TR[]
The Skull Crusher is one of the artifacts the Nerevarine may sell or donate to Torasa Aram at the Museum of Artifacts in Mournhold. She offers them 24,000 for it.
