A demonic Monarch Bat with yellow eyes and a flaming maul appeared to Bruce Wayne at age six when he fell into the network of caves under Wayne Manor. Whether this was a legitimate phenomenon or the hallucination of a frightened child is unclear. While the other common bats flew away the Monarch Bat stood its ground to confront the boy.
Later after a car crash, Bruce retreated to his father's study and contemplated bleeding to death. Another large bat with a normal appearance burst through the window and perched the head of a bust. Reminding him of his childhood encounter with the Monarch, he rang the service bell to call Alfred to his aid, inspired by the animal.
As an older man Bruce's flashbacks and nightmares of the Monarch Bat began to increase. After being defeated by the Mutant Leader, he returned to the section of the cave to encounter the specter again.

The Monarch Bat in a deleted scene from Batman Forever
- The Monarch Bat appears in flashbacks in Batman Forever. The bat is depicted similar to its comic incarnation, with it appearing to a young Bruce after he fell into the future Batcave. It also appears to manifest to Edward Nygma after he is defeated by Batman. In a deleted scene, the Bat appears to an amnesiac Bruce as he re embraces the Batman identity. The Monarch Bat was primarily created through the use of puppet.
