Astarion can be chosen as the player's character during character creation. His race, class, appearance, and background cannot be changed. All other details can be customized, including Ability Scores, Skill Points, etc.
If not chosen as the player's character, Astarion may be encountered on the Ravaged Beach and recruited to join the party as a companion.
