While Nickelodeon and their programming blocks have gone smoothly with their screenbugs and graphic identification, occasionally there have been times with things have gone haywire. This work-in-progress list details times Nick, as well as Nick at Nite and Nick Jr., have had errors with their screenbugs, split-screen credits, graphic identifications, and sometimes even the shows themselves!

Date Description Example
January 24, 1986 As Nickelodeon returns to an airing of Dennis the Menace, a channel slate is instead displayed for nearly two minutes before switching to a partially-ran bumper. DTMF tones can be heard numerous times as the slate is on-screen.
November 4, 1996 During the 8:00pm airing of the Hey Arnold! episode "Heat / Snow", a black screen is shown instead of a commercial bumper after the break prior to the beginning of "Snow".
December 1997 After a commercial break during an airing of What Would You Do?, a black screen was shown for 6 seconds. This was followed by a production slate for a "Garden" bumper, and then a few frames of said bumper, which all appeared in rapid-fire succession, before the episode abruptly cut in.

February 10, 1998 During the first half of the 7:00am airing of the Rocko's Modern Life episode "No Pain, No Gain / Who Gives a Buck?", the Fat Oval screenbug appears highly transparent to the point of near-invisibility. Midway through the first segment, the opacity is then adjusted on-air, finally returning to its usual appearance.
March 16, 1999 A commercial bumper accidentally played before the 8:30am airing of Rugrats, causing the first couple seconds of the show's intro to be cut off.
May 20, 2000 Prior to the 1:00pm airing of KaBlam!, a bumper for SpongeBob SquarePants is mistakenly shown, followed by the first four seconds of said show's intro. Eventually, the screen cuts to black before KaBlam! starts, albeit with the first couple seconds of the intro cut off.
October 14, 2000 During the premiere of The Amanda Show "Episode 214", in the middle of the "Moody's Point" segment, a slate for the screenbug briefly appears.
January 27, 2001 During the 10:00am airing of the SpongeBob SquarePants episode "The Chaperone", it was broadcasted without a screenbug with the rating bug lasting on-screen for over half the episode's runtime before finally disappearing near the end.
February 4, 2001 During the 10:00am airing of the SpongeBob SquarePants episode "Texas", the Rugrats 10th Anniversary screenbug was accidentally shown briefly at the beginning of the broadcast before switching to the correct bug.
February 26, 2001 The Rugrats 10th Anniversary screenbug is accidentally shown during the 5:00pm airing of Rocket Power.
April 20, 2001 During the premiere of the Invader ZIM episode "Germs", before the animated banner for the 2001 Kids Choice Awards premiering the following night, a slate for said animation accidentally pops up for several seconds. The screenbug disappeared for the rest of the episode.
December 26, 2001 At the end of one of today's airings of the Rugrats episode "A Rugrats Kwanzaa", the unedited credits played for 5 seconds before being interrupted with split-screen credits. Furthermore, not only was Gábor Csupó's first name misspelled as "Garbor", but the credits shown were for "All Growed Up" rather than those of the aforementioned episode.

March 20, 2002 At the beginning of the 7:00pm airing of Hey Arnold!, the rating bug lingers throughout nearly the entire intro, even continuing into the start of "Steely Phil".
May 27, 2002 Today's 3:00pm Nicktoons Summer Beach House airing of the SpongeBob SquarePants episode "Jellyfishing" accidentally used the cloud Nick.com screenbug. It gets replaced with the proper Nicktoons Summer Beach House bug about a minute into the episode.
May 28, 2002 The Nicktoons Summer Beach House screenbug was accidentally used during the first few seconds of the 5:00pm airing of the SpongeBob SquarePants episode "I'm Your Biggest Fanatic".
July 9, 2002 The Nicktoons Summer Beach House screenbug was accidentally used during the 5:00pm airing of the SpongeBob SquarePants episode "No Free Rides / I'm Your Biggest Fanatic", despite the block having ended for the day by then.
July 20, 2002 During the 7:30pm airing of the SpongeBob SquarePants episode "Rock-a-Bye Bivalve", an unknown Nick Jr. screenbug without proper chroma keying is shown during the last few minutes of the episode.
August 19, 2002 The Nicktoons Summer Beach House screenbug was accidentally used during the 5:00pm airing of the SpongeBob SquarePants episode "Pizza Delivery / Home Sweet Pineapple", despite the block having ended for the day by then.
September 10, 2002 Today's Slime Time Live airing of the Rugrats episode "Mommy's Little Assets" was suddenly interrupted by split-screen credits midway through with the sun Nick.com screenbug still intact. This lasted for about 5 seconds before the episode returned.
Today's Slime Time Live airing of the Jimmy Neutron episode "Normal Boy / Birth of a Salesman" had its split-screen credits completely interrupted by a host segment. This lasted for the entire credits duration, though a split-second of the split-screen credits (albeit with a blank promo box) can be seen before the production endcards.
October 3, 2002 The split-screen credits for the 6:30am airing of the Doug episode "Doug's Treasure Hunt / Doug's Brainy Buddy" ran a 15-second promo claiming that Sunday Movie Toons would be "tomorrow at 12:30pm," despite the fact that October 3 was a Thursday, and even if it were a Saturday, Doug never aired on weekends during this timeframe. After the promo ended, there was no other promo for the remainder of the credits, leaving the promo box blank.
October 11, 2002 During the 6:30am airing of the Doug episode "Doug Meets RoboBone", the skull Nick.com bug was seen for the majority of the episode (along with the episode before it "Doug on the Trail") before switching to the leaf Nick.com bug.
October 13, 2002 The SLAM! screenbug is shown during the 9:30am airing of the SpongeBob SquarePants episode "Snowball Effect / One Krab's Trash", despite the block not beginning until 4/3c.
December 2, 2002 Break bumpers for SpongeBob SquarePants were shown during the 8:00pm airing of the Rugrats episode "A Rugrats Chanukah".

August 20, 2003 After the split-screen credits following the 6:00pm airing of the Cousin Skeeter episode "The Not So Great Outdoors", a copy of the live segment was briefly shown on the left-side of the screen, before cutting to a still of the intro to Hey Arnold! After two seconds, the live segment resumed.

September 12, 2003 During the split-screen credits for My Life as a Teenage Robot, a squished copy of the credits template appeared in the promo box, causing there to be no promo audio for the entire credits duration.
December 12, 2003 During the 4:00pm airing of The Fairly OddParents episode "The Big Problem / Power Mad", the Nick@Nite logo is accidentally shown.
February 14, 2004 During the 8:00pm airing of the SpongeBob SquarePants episode "Just One Bite", the uncut version where Squidward tries to reach the Patty Vault was accidentally shown.

August 11, 2004 No promos were played at all during the split-screen credits for Rugrats, resulting in the template being left blank for the entire credits duration.
August 14, 2004 Instead of announcing more Jimmy Neutron after 11:30am, Saturday Morning Nicktoons incorrectly runs a bumper advertising a second airing of My Life as a Teenage Robot. The latter would only air once that morning at 12:30pm.
April 2, 2005 After the premiere of The Fairly OddParents episode "Blondas Have More Fun! / The Five Days of FLARG" and before the premiere of the Nickelodeon Kids' Choice Awards 2005, the split-screen credits ran into a technical fault. The credits first appeared without a promo on the right side and with The Fairly OddParents theme music playing for about 9 seconds, obscuring the live promo preceding the Kids' Choice Awards, but with the announcers' voices intact. At the end, 2 transparent versions of the production endcards (one squished to the left hand side and one sized normally) can be seen on top of the live promo, and once these logos disappear, The Fairly OddParents theme is once again heard for about 5 seconds, completely obscuring the announcers' voices this time.
January 3, 2006 During this day's airing of Drake & Josh episode The Bet at 6:00pm during Drake & Josh's Personal Picks, the abstract crystals Nick.com bug was accidentally shown, after the usual screenbug was missing for most of the episode. Shortly thereafter, the crystals bug was switched to a shadowless variant of the 04-06 .com splat, which lasted a bit into a commercial break before being removed from the screen manually by a broadcast engineer.
March 17, 2006 After the premiere of the Danny Phantom episode "King Tuck", during the split-screen credits, the original unedited credits for the show played instead of the split-screen ones, causing the "Winter 2005" split-screen template and promo to appear over the Danny Phantom credits. As a result of this error, the Danny Phantom credits theme and the promo voiceovers overlapped with each other. Even after the template and promo disappeared, the "Up Next" indicator for more Danny Phantom remained on-screen for another 5 seconds while the unedited credits continued to play.
June 29, 2006 The TV-Y7-FV rating icon accidentally appeared during the split-screen credits for the 2:00pm airing of the Danny Phantom episode "13". In addition, the split-screen credits template with an empty promo box remains intact over the production endcards and first second of the next Danny Phantom episode.
October 3, 2006 During the 4:30pm airing of The Fairly OddParents, the screenbug was not shown during "The Big Scoop."
March 31, 2007 The 2006 "Arrow and Flowers" template is used during the split-screen credits despite only being meant as a guideline. In addition, the promo bleeds into the production endcards for a split-second. Also, the credits shown are for the wrong episode of SpongeBob SquarePants; the credits are taken from "Patrick SmartPants / SquidBob TentaclePants" when the episode that actually played was a standalone showing of "Waiting".
May 6, 2007 The 2006 "Arrow and Flowers" template is used during the split-screen credits despite only being meant as a guideline. In addition, the promo bleeds into the production endcards for a split-second.
June 16, 2007 The premiere of the El Tigre episode "Yellow Pantera / Rising Son" had numerous technical errors, including going to commercial during the title card for "Yellow Pantera", having no on-screen bug during random parts of both segments, and having the split-screen credits play during the "Yellow Pantera" episode. Due to the credits airing in-episode, at the end of the second segment the screen goes black and starts to play the show's normal credits, before stopping and the 11:30am episode of Jimmy Neutron begins.
September 2007 During the split-screen credits for an airing of The Fairly Oddparents, an unusual split-screen credits template can be seen for a split-second at the end.
November 22, 2007 The 2006 "Arrow and Flowers" template is used during the split-screen credits despite only being meant as a guideline. In addition, the promo bleeds into the production endcards for a split-second.
July 7, 2008 During the Nonstop Five at Five block, after the SpongeBob SquarePants episode "Banned in Bikini Bottom" and before The Fairly OddParents episode "Genie Meanie Minie Mo", the Nonstop Five at Five bumper and The Fairly OddParents intro used a slightly larger version of the standard split-screen credits placing format instead of the Nonstop Five at Five placing format, causing there to be a half of empty space in the box. In addition, the end of The Fairly OddParents intro overlaps the split-screen credits template for a split second.
January 3, 2009 Today's airing of True Jackson, VP used split-screen credits from the wrong episode. The credits used appear to be from "Pilot" instead of the episode that actually aired that day, "The Prototype". In addition, after the promo ends, a blank promo box can still be seen for a split-second during the production endcards, and a copy of the endcards can strangely be seen in the bottom left corner for a split-second.
August 8, 2009 The 2006 "Arrow and Flowers" template is used during the split-screen credits despite only being meant as a guideline. In addition, the "Up Next" indicator bleeds into the production endcards for a split-second.

September 23, 2009 Presumably right after the 6:30am airing of the The Fairly OddParents episode "Smart Attack / Operation F.U.N." and right before the SpongeBob SquarePants episode "Welcome to the Chum Bucket / Frankendoodle", the 2009 rebrand split-screen credits template (with no "Up Next" indicator in the bottom corner) and production logo endcard was used 5 days before its intended release, presumably as a test.
September 26, 2009 Throughout the remainder of the day during where the closing logo endcards would take place in split-screen credits, the then new Nickelodeon Productions endcard made for the 2009 rebrand showed up presumably by accident two days before its intended release (similar to what happened on September 23), with audio from the "Lightbulb" endcard logo being heard in which is sourced from the "Credits Guideline" template for the episode print of the selected program.
October 7, 2009 After the 7:00pm airing of the iCarly episode "iGot Detention", during the split-screen credits the old placing format used in pre-rebrand split-screen credits was used for the promo, causing there to be space in it.

December 8, 2009 The TV-Y7 rating bug was accidentally shown during the split-screen credits after Fanboy & Chum Chum.
December 17, 2009 After the 7:00pm airing of the iCarly episode "iDate a Bad Boy", during the split-screen credits the 20-second-long "Up Next" indicator for Malcolm in the Middle used for Nick@Nite split-screen credits is shown accidentally instead of the regular 30-second one Nick uses when transitioning to Nick@Nite. Considering the graphic being 20-seconds long, it disappears 20 seconds in with no indicator graphic used afterwards, leaving it empty.

October 29, 2011 After the 7:30am airing of the Rugrats episode "Curse of the Werewuff", the split-screen credits and template appeared too early. As a result, the credits template switches from the SD version to the "Prototype 2009 Rebrand" split-screen credits template during the last 2 seconds (in addition, the remainder of the promo is unable to fit in the "Prototype 2009 Rebrand" template's box), and the TV-Y7 rating icon for the following episode of Fanboy & Chum Chum appears on the Nickelodeon Productions endcard.
