The Piano Truck is an oversized truck driven by the Truck Driver to transport large shipments of Pianos. It's so wide that it covers all highway lanes going both ways, creating a lot of traffic problems and destroying bridges.

Numbuh 4 was forced by his parents to learn how to play piano, so in an effort to avoid this he got his teammates with to raid the truck carrying all the pianos. The landed on the truck and cut a whole through the roof to enter, where they learned the truck were not carrying "Type P Kid Torture Devices", but just regular pianos. Before they unloaded all the pianos, Numbuh 4 kicked the backdoor open and was certain that he knew how to handle the job until he saw the rest of the shipment run over him to fall out of the back, resulting in him being confined to a wheelchair. They later returned to the truck once they learned that a single piano was still aboard since Numbuh 4's stragety hadn't got all the pianos. A failed distraction by a hamster resulted in the piano falling over the rest of Sector V, putting them in wheelchairs as well (Operation: P.I.A.N.O.) since the driver would fall the hamster's fake stories of being injured and hurt and distracting him long enough to forbid him to see the rest of the team go into the back of the truck to destroy the last piano.
A piano from this truck was on the list of items for the great scavenger hunt made by Numbuh 74.239, with the prize being the Delightful Children From Down the Lane's birthday cake. The Six-Gum Gang held the Truck Driver at gunpoint while unloading a piano, which landed on and crushed Runt (Operation: I.N.T.E.R.V.I.E.W.S.).
