Earth-18 is one of the many realities in the Multiverse. It is set in the Wild West era and, at least, one Lazarus Pit exists on this world.
History
At some point in time, Jonah Hex became the owner of a cave in North Dakota with a Lazarus Pit inside.[1]
Earth-18 was depicted on a map of the Multiverse which Jay Garrick showed to Barry Allen.[2]
During the Crisis, after the Anti-Monitor had destroyed countless Earths, Earth-18 housed the last functioning Lazarus Pit in the Multiverse. When Sara Lance, Mia Smoak and John Constantine came to this universe searching for the Pit, Hex confronted them: he was quickly overpowered by the heroes and Sara also scarred him. The heroes, then, revived Oliver Queen thanks to the Lazarus Water.[1]
Later that day, this universe was annihilated by the Antimatter Wave that the Anti-Monitor had previously unleashed into the Multiverse.[3] After the Spectre and the Paragons rebooted the Multiverse, Earth-18 was restored along with all the other destroyed realities.[4]

Trivia
- The Earth designation and the 19th Century-based setting of this universe are inspired by the Pre-Flashpoint Multiverse Earth-18 as well as the Post-Flashpoint Multiverse Earth 18 which are both set into the Wild West era and the home of the Justice Riders.
- According to Marc Guggenheim, Earth-18 was originally designed to be the setting of Titans before it was changed into Earth-9.[5]
- The Jonah Hex of this reality is a doppelgänger of Earth-1 Jonah Hex that Sara Lance met multiple times in the past.
- During their fight, Sara gives Hex a version of the iconic scar that his Earth-1 counterpart also has.
