The Myconid Bloom-Sac is in the inventory of the Myconid Elder in the Underground River Entrance. The Myconid will drop the item if killed. Alternatively, if Jaheira is in the party, she can translate. Bellowgulp Bluefingers can also appear and translate. Both translation interactions can result in an exchange for the bloom-sac. The item summons 1d4 Red Myconids. Ferrusk can trade the bloom-sac for the Slime Sickness Curative, which is needed for the Slime Sickness quest.
