- The baseball player outfit could be a nod to the Japanese Pro-League baseball player Akira Nakamura, who shares a name with the hospital director.
- Though not possible to hear in normal gameplay due to it being a disguise that turns the entire map into a hostile area, there are dialogue options from frisk point guards that comment on the outfit.
- One of them has 47 be referred to as Ichiro, the mononym given to Japanese baseball player Ichiro Suzuki.[1]
