When Roger runs out of a truck and back toward the mall, one
particular zombie in a red-and-black striped shirt gets out of character and decides to tuck in his shirt.
The blood on Roger's face and hands in the truck scene suddenly disappears after he turns around. When he drives off and starts running zombies over, the blood is back.
As the Trivia Section notes, shooting of the movie was done during the Christmas Season and was suspended during the holidays to avoid the cost of re-hanging decorations. As a result, JC Penney (the store of which they keep going in and out), has a sign in the window saying "After-Christmas Sale" later in the movie.
A distinctive zombie in a nurse's uniform survives right up until the final attack on the refuge, all the more remarkable considering she was seen in the mall during the lock-down (while all of the entrances were already blocked with trucks) and must therefore have been in there during the mass extermination.
During the sequence when the group locks down the mall and eliminates all of the zombies trapped inside, almost all of the "Featured Zombies" including Sweater Vest Man, Nurse, Rifle Head, Fat Swimsuit Man and Red Sweater Scarf Girl are all locked inside before the purge begins. Later at the end of the film, all of these featured zombies are still alive and re-enter the mall when the raiders attack.
When killing Zombies in the basement of the project, Peter
fires seven shots out of his revolver. Although revolvers can hold anywhere from 5-10 rounds, Peter's revolver only held 6 rounds.
Berkley (sic) is misspelled when it runs across the screen on a news report at the beginning of the movie
Near the beginning of the movie, the fully-loaded helicopter (Bell JetRanger II), departs the TV station sometime after 9pm. The helicopter would need refuelling within 4hrs, yet they're shown to have flown all through the night before finding a place in the early morning.
When Peter is inside the airport office with the plaid shirted zombie approaching him from behind, he jumps at the sight of the zombie and sees Stephen in the background aiming at him and the zombie. He jumps out of the way as Stephens first round misses and ricochets. Stephen then fires 2 more consecutive shots within a second of each other. Stephen has a Marlin Model 983 .22 caliber bolt action rifle. It would have been impossible for Stephen to work the bolt, aim and fire within a seconds time. Only with a semi or an automatic weapon would he have been able to fire that fast.
Given our protagonists descend a stairwell to a hallway that eventually leads directly to the 2nd floor of the mall, it would indicate the mall has a 3rd story. However, the Monroeville Mall is only 2 stories high.
Right after Peter and Stephen first pull up in the trucks, before Peter tells Stephen "c'Moon hot shot, c'Moon," you can see a zombie extra in a suit and tie with absolutely no zombie make-up on.
The Eye patch wearing scientist Millard Rauch, played by actor Richard France, is described as being from the "OEP" which means the Office of Emergency Preparedness. But this agency had been dissolved by 1973 and rolled into Housing and Urban Development, five years before the time frame of the film. Disaster response would only be assigned to FEMA (Federal Emergency Management Agency) in mid 1978, mere months before the film's first release (which was overseas).
It's shown that after the guys put up a wall to block off the stairway, their living quarters upstairs was still not furnished. So it would follow that all the stuff brought up thereafter would have to fit through the ducts, which would be impossible with beds, furniture, various appliances, etc.
The mall montage scenes in which they loot the stores occur before the wall is finished. Although not shown, that does not mean that the survivors didn't bring furniture to the back area, and set it up later after the mall was secure.

52min into the film, Stephen is checking the portable TV for any news updates. Roger asks him what the time is. He says it's not quite 9 yet. Firstly, Roger is wearing a watch as he does all through the film so he wouldn't even have to ask. Secondly, Stephen's watch appears to show the time of 6:45.
Neither watch is clearly visible, and both men were wearing mechanical watches. Many mechanical watches at the time required regular, manual winding and given their situation they may have neglected to do so.

When a one-armed female zombie grabs onto the back of the hero's car as they drive through the mall, she is shot through the eye. When blood comes pouring down, it can be seen that the zombie has a mustache. (For the main part of the zombie, a woman was used, but stunt man Taso N. Stavrakis took over for the part of being dragged behind the car.)
When Roger is supposedly putting gas in the helicopter, he actually has the nozzle stuck in a footstep used for climbing up and inspecting the engine. The step has a spring-loaded cover that swings inward. The gas nozzle is simply stuck inside the cover.
When Roger hits a zombie with his truck, the trampoline the stunt man bounces off can be seen briefly.
At the airfield, while Roger is filling up the helicopter, a zombie climbs over some crates to get him. As he gets to the top, the blades of the helicopter chop off the top of his head. This causes blood to ooze out of the top of his head and down his body. The pump tube for the fake blood can be seen coming from up behind the crates and up the zombie's pant leg. Additionally, when viewed from behind, another tube can be seen attached to the back of the zombie stunt actor's head as it/he falls forward.
The first zombie, who appears in a kitchen in the tenement building, has a damaged leg (missing the foot) that is noticeably longer than the leg that is intact.
In the Director's Cut, the stairwell argument is edited such that when Fran says, "Stephen, for God's sake!" her lips are actually saying, "We've got to get up on the roof!" Compare this segment with the theatrical cut.
When Peter talks to Stephen at one point, his hand blocks his mouth. Moving away for a few seconds, however, it can be seen that Peter's lips do not match the words on the audio track.
Half hour into the film, just after Peter points his rifle at Stephen to prove a point, he helps him up, pats him on the shoulder. Then you see Peter's mouth moving as he says something to Stephen but no ADR was done, you only hear the helicopter.
When Peter grabs and throws the two zombie children onto the couch at the airport before shooting them, a hand can be seen entering the top right-hand portion of the screen.
In the grocery store where Stephen and Peter are grabbing the loaves of bread, in the reverse shots of Peter talking to Steve you can see the silver microphone zeppelin in the left side of the screen.
When Blades sprays a seltzer bottle at the camera and soaks two zombies, a reflection of the optical flat (the piece of glass to keep the camera safe from effects) can be seen in the middle of the shot.
When Peter, Roger, and Stephen are attacked while trying to get the shopping
cart upstairs, (the part were Peter throws a Zombie over the balcony into the fountain) a crew member's hands can be seen preventing the Zombies that are attacking Stephen from bumping into the camera when he knocks them away. This is visible only on full-screen prints, however.
In the wide shot of the truck driving across the parking lot after Peter and Roger have placed the first truck, there's a cameraman visible on the very edge of the left side.
Peter and Roger are cops in Philadelphia, but when they drive to the docks, their police car is that of the Pittsburgh Police Department.
When flying the helicopter, over what they say is near Johnstown, people in the town below are shown, with a Monroeville fire truck there.
When the heroes are shopping at the mall and Peter & Flyboy are getting the huge loaf of bread (where Peter says 'Maja!') a mic is visible on the very edge of the left side of the screen.
The heroes use walkie-talkies when they're staying at the mall, even until the bandits loot the place. Yet, when the heroes are moving the trucks to block the entrances, they do not have them. Roger's death could have been avoided had Peter used one and Flyboy had the other.
Purging the mall of zombies, the characters put their bodies in the freezers with the fresh food. It's still rotting flesh that can contaminate unused food.
Fran calls Stephen David (the actor's name) in the news station at the beginning.
